First, your part.

Complete the parent or guardian consent form below. It covers:

• Permission for her to participate
• Recording and replay consent
• Emergency contact information
• ARC's program scope and communication limitations
• Acknowledgment that her participation must be voluntary

It takes approximately five minutes.

Then, hers.

Once your form has been submitted, she will receive her own private email with a link to:

• Her participant agreement
• The group privacy and no-sharing agreement
• Her recording and replay acknowledgment
• ARC's program scope and communication limitations
• Her short intake

You do not need to forward anything. Her invitation will be sent directly to her using the contact information you provide in the consent form.

Please give her privacy while she completes it unless she asks for help understanding something or needs accessibility support.

What she writes in her intake comes to me and is not routinely shared with you. A meaningful safety concern may require additional action, as explained in the consent form and participant agreement.

Both parts must be completed by September 30.

If she says no

That is a real answer. It is one of the reasons her agreement is separate from yours.

If she decides not to participate, let me know at [email protected] and you will receive a full refund. Nobody needs to persuade her to join.
