I am the parent or legal guardian of the participant named above, and I give permission for her to take part in ARC for Young Women.
I understand that her participation must be voluntary. She will complete her own agreement separately, and if she declines, she will not be enrolled and I will receive a full refund. I have not required her to participate against her wishes.
I understand that ARC is an educational and coaching program. It is not therapy, counseling, psychotherapy, medical care, mental health treatment, or crisis intervention, and it is not a substitute for any of those. Katie Adams is not providing services as a licensed mental health or medical professional.
I understand that I will not receive routine reports about what she says, writes, or shares in ARC, and that I will not have access to session recordings. I will receive program logistics, scheduling, and general information about what each week covers. I understand this limitation exists so that she can reflect honestly, and I agree to it.
I consent to the live sessions being recorded and made available to her cohort for private replay for twelve months. I understand recordings are not provided to parents, guardians, or the public, and I agree not to request, access, copy, or share them.
I understand that participants agree not to repeat another participant's personal information, record sessions themselves, take screenshots, or share another person's story. I understand that absolute confidentiality cannot be guaranteed in a peer group.
I understand that if Katie reasonably believes she or another person may be in significant danger, or that abuse, neglect, exploitation, or another reportable concern may be occurring, Katie may contact me, an emergency contact, a qualified professional, an emergency service, or an appropriate authority, and may be legally required to do so.
